2019 8月にはCOVID19 が日本国内へ侵入し、蔓延していた可能性が高いとの研究結果を得ましたので報告致します。 6park.com
一人でも多くの世界中の尊い生命を救いたいという我々医師、医療従事者達の強い理念から今回、100名以上の同志達で、MS Task Force for COVID19 (以下MSF)を結成しております。MSFでは今回のCOVID19による感染がすでに昨年秋、日本国内で第一波が蔓延していた可能性があると考え、仮説検証試験を行なっております。厚生労働省の基準に則って、2019 9〜2020 2月の過去の検体による後ろ向きコホート研究、および、2020 4月から10月にかけての日本全国多施設検体による前向きコホート研究を、中国FDAおよび欧州CEの国際承認のある、イムノクロマト法による IgG IgMキットであるイノビータおよび、ELISA法によるIgGキットを利用して、現在仮説検証試験中です。2020 5/5 現在、以下の通りの前向きおよび後ろ向きコホート研究の途中結果を得ておりますので報告致します。今後の結果については追って詳細結果をまとめた上で学会発表等を行い、その他MSFとして発表致します。既に先行前向き試験の結果は学会へ提出、二次査読中です。 6park.com

② 前向き　先行試験　(使用キットは　Kurabo Japan)(国内多施設試験) 6park.com
54例中8例（14.8％）においてIgG抗体陽性を認めた。IgG抗体陽性例8例のうち感冒症状を認めた症例は4例（50.0%）であった。うち1例は都内在住　2019 10/28 発症40代女性医師。2ヶ月ほど続く原因不明の咳、副鼻腔炎症状が持続し、2020 4/3 IgG陽性、IgM 陰性であった。また東京だけでなく、広島、高知、大分、長崎、の各県でもIgG抗体陽性例を認めた。 6park.com

2019.9/24 凍結血清でのIgG陽性例は2019.8/20 から数年症状のなかった長引く喘息発作症状を認めており、ここが発症と考えられます。今後症例発表を行う予定です。遅くとも昨年2019 8月には日本にCOVID19が上陸しており、昨年秋に感染が蔓延していた可能性が高いと考えます。また、IgG抗体の低下に伴う二度目の感染を起こすと考えられ、2020 3月頃より第二波が開始していると考えます。今後、大規模な抗体検査の実施が望まれます。また、IgG陽性者は今後、ELISA法により半年間の観察を行い、抗体がどの期間まで持続するか、数値で明らかにする予定です。また、世界中で昨年6月以降の検体での後ろ向きコホート研究をやるのが最重要だと考えます。 6park.com
MSFでは第二波で重症化するメカニズムが隠れていると考えています。またパンデミックでの重症化の主病態をサイトカインストームによるものだと疑っています。サイトカインストームのメカニズムはまだはっきりと解明されていませんが、間質性肺炎や血管炎の誘引になると考えられています。海外では小児の川崎病類似症状の出現や20-30%の症例で血栓症が出現しているとの報告がある事からも血管炎からの血栓症の出現と考えるのが病態も説明しやすいと考えます。肺その他の病理標本からもサイトカインストームを示唆させる所見が出ています。また、二度目の感染で重症化する可能性がある為、IgG抗体陽性者こそ、今後の感染時に重症化リスクが高くなると考えています。 6park.com
また、治癒症例において、再感染事例の報告が多発しております。現状ではIgG抗体の有無だけでは説明が出来ず、中和抗体の獲得が出来ているかの研究には今後時間を要します。まずはしっかり感染を防御する事が最優先であると考えます。今後、疫学研究結果をまとめ、詳細検討の上、学会発表などを通して、更なるメカニズムの解明、治療法の検討に努めていきたいと考えます。 6park.com
